i don't think these were mistakes necessarily. they may have been intended by the wachowski brothers to give the film the sense of doubles, i.e. trinity face up, then down; mr. anderson's hands in front, then back; guns there, then not there. this gives the film a sense of duality, because the protagonists in the film move between two worlds, so it seems consistent with the themes in the film. this video was very interesting though.

sadly this doesn't list the one scene that always makes me twitch.. Lobby gunfight and the squib'd foam pillars. There's a clip at the start of the lobby scene with Neo and Trinity where Neo is running behind a pillar and you see bullet impact along the walls and post. At one point, the squib or impact goes off and you see a chunk of the 'marble pillar' hanging. It reveals that it's some foam/padded material which sadly takes away from that entire scene for me.
